HealthCare Support is actively seeking a PT Home Health for a 13-week contract. Located in Kokomo, Indiana, this area is known as the “City of Firsts,” taking pride in its variety of historical museums, art glass tours, and intriguing roadside attractions.

Requirements of this PT Home Health Role:

• Active PT license
• Home Health experience required
• 1+ year of experience within this specialty
• BLS Certification from the American Heart Association PT Home Health

Assignment Details for this PT Home Health Role:

• Guaranteed Hours: 40 hour schedules
• Five 8 hour shifts – days
